Plenary I: Bridging Practices and Communities: A Week in the Life of a Practice Facilitator in Rural Colorado

The High Plains Research Network (HPRN) is a practice- and community-based research network that works with 53 primary care practices, 16 hospitals, in the 16 county region of rural eastern Colorado. Ms. Zittleman and Ms. Sutter will describe the HPRN, its history of engaging communities in research, and how practice facilitation has been woven into its research and quality improvement efforts. They will preset examples of projects that bridge a broad range of community organizations and practices, successful strategies, and reflections on improvements. 

 

PLENARY II
Cross-Community Facilitation for Social Determinants of Health Screening and Referral in Oregon 

Steven Brantley, MPH, CCRP
Practice Facilitator & Research Associate, ORPRN

The Oregon Accountable Health Communities (AHC) study is designed to screen and refer more than 75,000 Medicaid or Medicare members annually for social determinants of health. Steven Brantley, a practice facilitator for the Oregon Rural Practice-based Research Network (ORPRN), will describe how their team has engaged nearly 50 primary care clinics, dental offices, social service agencies, community and other partners in this work. He will describe the background and context of AHC and outline facilitation strategies used to engage study participants and partners.
